Ejaculation is the spurting out of semen from the penis, often called coming. It is usually a very pleasurable event, and because it feels good, and is the fertile part of manhood, when it goes wrong it is has a very bad effect. It can undermine a man's sense of identity and manhood and cause Fear of Sex, depression, and stop him making relationships. It is important to get help, as most problems with ejaculation can be improved, and sometimes may be the first sign of a medical illness.

There are 6 main ejaculation problems that men can have.

  1. Premature Ejaculation, which is the most common of the ejaculation problems. This means a man cannot control how fast he comes. He can come before touching the penis, or at the moment of penetration or very soon after.
  2. Retarded Ejaculation, when it can be very slow to come, and in some cirumstances, impossible.
  3. Retrograde Ejaculation, when the semen goes into the bladder. This is usually caused by medication or illness. The main sign is that no semen comes from the penis with orgasm, and the first urine passed after orgasm is cloudy. See a doctor if this happens to you, as the cause is usually medical.
  4. Dry Orgasm, when the man feels orgasm but there is no semen. Some men strive to achieve this, but others find it a problem. See a doctor first.

Squeeze Technique
To make this technique work, you need to be prepared to set aside private and uninterrupted time three times a week. Don't be tempted to give up if after a few tries nothing seems to be changing. It can take a while to get the hang of it. Often it can take 30 attempts to learn the moment to squeeze, so giving up after 29 would be a mistake!

  1. The place to squeeze is just below the glans, the rounded part at the top of the penis. If you have a foreskin, pull it back so the glans is uncovered.
  2. Stroke and caress your whole genital area. This consists of the glans, the shaft of the penis, the testicles or balls, the area behind the balls which is called the perineum, and the anal rim. 5 parts altogether.
  3. Practice touching and pleasuring all these parts so you are very aware of how each part feels in the stages of arousal before you are going to come.
  4. Now increase your pleasure to a high state of arousal, and when you reach the point when you think you will come, squeeze the penis hard and hold ot for 20 to 30 seconds. The erection may go down, but you can get it back. If you were too late with your squeeze, try again later and squeeze a little earlier in your arousal.

James was 30, and worried. Whilst he could ejaculate easily with masturbation, with penetration it was impossible. He had wondered if there were deep seated problems causing it, if he were unconsciously afraid of or angry with his partner, but could not see why that might be so.

Medication for Premature Ejaculation

There are two medical treatments which are at present most often used to help men with *Premature Ejaculation*. These are Clomipramine and Paroxetine. There are several others, but these are the commonest.

Clomipramine may be taken in two ways.

  1. As a 25-50 mg tablet one hour before intercourse.
  2. Every day in a small dose of 10 mg, which is increased by 10 mg each week until improvement shows, but not more than 60 mg. After three to six weeks at that level, it is then reduced each week by 10 mg.

This drug is one of a group, the tricyclics, which are used as antidepressants, but in a dose below what you would have for depression. Because the dose is small, the usual side effects are less common, and wear off easily. This drug can work for up to 75% of cases. The premature ejaculation can return after treatment is stopped, and at times of stress and pressure, unless the underlying cause of the problem is resolved.

Paroxetine is another antidepressant. When it is taken in a dose of 40 mg/day, it can help a man get much better control of ejaculation. Rarely this drug (and others in the same group such as sertraline, and fluoxetine) can cause reduced sex drive or desire, or erection difficulties, when taken on a regular basis.

This is often called PE for short.If you feel you ejaculate too fast the first thing to consider is "How do I define too fast?" The best yardstick as to whether there is a problem about coming too fast is whether you and your partner find intercourse satisfactory for you both.

One report has suggested that most men ejaculate within two minutes of penetration. However, some men find they come before they have penetrated, and maybe before they have an erection.

 

The key factor is whether or not you can decide when to come.

  • reduced frequency of sexual intercourse, such as during absence of a partner
